We specialize in Neuromusculoskeletal and Sports Medicine. We decided to add Medical Marijuana service to our practice to provide alternative options to our patients. What sets our practice apart from other Medical Marijuana providers is that we offer other treatment options. We treat patients as a whole, with a more comprehensive approach. Under the State of Florida Medical Marijuana laws, we will provide the necessary certification for qualified people after a face to face evaluation with our Physicians. Along with the certification process, we offer information and education to our patients as an alternative therapeutic treatment to pharmaceuticals.

HOW TO QUALIFY

All patients must follow these steps:

  1. Medical records collection. These documents are imperative for the patient’s initial evaluation.
  2. Schedule an appointment for Physician evaluation. Please bring a valid Florida Driver’s License or Identification card.
  3. After being evaluated and qualified by our Physician, we enroll you in the Florida Marijuana Registry Program.
  4. Apply online as a patient on the registry.
  5. Once the application process is completed, the patient will receive their Medical Marijuana Card.
  6. Patient’s holding a valid Florida Medical Marijuana Card will be able to legally obtain their medical marijuana at licensed dispensaries.
  7. According to the law, patients must be re-evaluated and re-certified each 210 days.

WHO QUALIFIES FOR MEDICAL MARIJUANA

Qualifying Conditions for Medical Marijuana in Florida:

  • Cancer
  • Epilepsy
  • Glaucoma
  • HIV
  • AIDS
  • Post-traumatic stress disorder (PTSD)
  • Amyotrophic lateral sclerosis (ALS)
  • Crohn’s disease
  • Parkinson’s disease
  • Multiple sclerosis (MS)
  • Medical conditions of the same kind or class as or comparable to those above
  • A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification
  • Chronic nonmalignant pain caused by a qualifying medical condition or that originates from a qualifying medical condition and persists beyond the usual course of that qualifying medical condition
